Late Registration

Bring the following completed forms:

Family Information Form, Contract of Enrollment, Release of Liability and General Agreement, Signature Sheet and Co-op Rules of Conduct.

Also, complete one Class Selection Form for each student. Then write out material fee checks to each teacher.

Bring all forms and checks to Late Registration.
